'Mike' John Michael Boaz

""Mike"" John Michael Boaz, born March 20, 1944 passed away October 22, 2012 at Kate B. Reynolds Hospice Home in Winston Salem. Mike was born in Louisville, KY to David and Georgie Clark Boaz. He is survived by his wife of 42 years, Maxine Newman Boaz; two sons: Michael ""Shane"" Boaz(Angie)of Winston-Salem and Jason Boaz(Amy)of Lake Charles, LA; five grandchildren: Logan, Ryan, Nichole, Gabe and Angel; one brother, David (Barbara) Boaz; and sister, Sue Atkins. A memorial service will be held Wednesday, October 24, 2012 at 11:00 A.M. at Beck's Baptist Church. The family will receive friends following the memorial service. The family request no flowers and that memorials be sent to Hospice and Palliative Care Center, 101 Hospice Lane, Winston-Salem, NC 27103.
