Charles Polk

Mr. Charles Olin Polk, 77, of Winston-Salem passed away Sunday, December 2, 2018 at Forsyth Medical Center.  He was born April 19, 1941 in Forsyth County, the son of the late William Claude and Elma Polk.  Mr. Polk was retired from Wilco-Hess.  In addition to his parents, he was preceded in death by four brothers. Survivors include his wife, Mary Polk of the home and two stepdaughters, Janet Hawks of Kernersville and Deborah Carter and husband, Mark of Cottageville, SC.  The family will receive friends Wednesday from 6:00 until 8:00 PM at Hayworth-Miller Funeral Home on Silas Creek Parkway.  In lieu of flowers memorials may be made to The American Heart Association, 7029 Albert Pick Rd Ste. 200, Greensboro, NC 27409.  Online condolences may be made at www.hayworth-miller.com.
