Richard 'Rick' Curtis Smith

Richard ""Rick"" Curtis Smith, 52, of Clemmons, passed away Monday, May 28, 2012 at Forsyth Medical Center. He was born February 6, 1960 in Forsyth County to the late Dick and Toots Smith. Surviving are three sisters, Tammy Smith (Debbie Reynolds) of Winston-Salem, Robin McIntosh (Keith) of Midway, and Dawn Mitchell (Donnie) of Canton; a niece, Kristen McIntosh; a nephew, Ryan McIntosh; and his beloved pets, Sam and Missy. A memorial service will be held at 11:00 a.m. Saturday, June 2 at Hayworth-Miller Silas Creek Chapel. Memorials may be made to the American Diabetes Association, P.O.
